Setup
Client standing, feet together. Mark two points on the spine: one at the PSIS line (S2), one 15 cm directly above. This is the modified Schober test.
Cueing
Step 1: 'Bend forward as far as you can, reaching toward your toes, without bending your knees.' Step 2: Measure the distance between the two marks at end-range flexion. The distance should increase by 4-8 cm (skin distraction). Step 3: Return to standing. Watch for: knee flexion, weight shifting to heels, breath holding.
Measurement
Measure distraction in cm (end-range distance minus 15 cm). Normal: 4-8 cm increase. Less than 4 cm indicates restricted lumbar flexion.
